Cancelling your magazine subscription - sample webform 

Use this webform to let us know your name and mailing address. It usually takes a month or two before you stop receiving the magazine in the mail again, so be patient after you make your cancellation request.

You can also call the office to cancel. The number is 123-456-7890.  Please speak slowly and spell out the name of the subscription and the mailing address. It's important to spell out the names of streets because sometimes just speaking the name isn't enough to get the spelling correct on our end.  Thank you.
